We are seeking a compassionate and motivated Assistant Social Worker to join our team. In this role, you will support social workers in delivering excellent care and services to our clients. Your responsibilities will include assisting with case management, conducting client assessments, completing care plans and conducting reviews. Your work will ensure our clients receive the necessary support to enhance their quality of life, whilst maintaining their independence for as long as possible.

Job Purpose

To support and assist the Adult Social Care team in the provision of a comprehensive social care assessments, early intervention and reviews for the vulnerable adult population in Jersey alongside their families and carers. To support with delivering the day to day services, accessing and facilitating resources to manage risk and meet identified needs.

Statutory Responsibilities

Active engagement, participation and compliance with any other statutory responsibilities applicable to the role, as amended from time to time. This role is politically restricted. The job holder is not permitted to undertake political activity involving standing for election to the Government or as a Parish Constable, or publicly supporting someone who is standing for election or playing a public part in any political manner.

The main duties of an Assistant Social Worker are: - To holistically assess the social care needs of a service user, to include early intervention community support, providing ongoing care planning and review services proportionate to the role as agreed by their line manager. This may involve working alongside other health professionals and departments to ensure positive and effective MDT working practices are followed. Deliver appropriate, open, accurate and straightforward communication to service users, their families and carers, clearly explaining the responsibilities of the social work team and subsequent services that could support the individual. Where possible promote independence and provide relevant information to service users to enable them to make safe and informed decisions regarding their care needs and daily routines. This may require the Assistant Social Worker to advocate on behalf of a client, supporting them to appropriate meetings or visits in line with their care plan objectives. To assess and identify appropriate funding and support to access community resources to meet the identified needs of the service user e.g. Long Term Care benefits, income support and charity funding. The Assistant Social Worker is required to manage their own caseloads in consultation with their line manager and is required to work with a degree of autonomy, prioritize cases, manage their time effectively, and make appropriate recommendations in the interests of the service users’ health and well-being. Qualifications

Qualifications Please state the level of education and professional qualifications and / or specific occupational training required.

Be educated at NVQ level 3 in Health and Social Care or equivalent.

Knowledge This relates to the level and breadth of practical knowledge required to do the job (e.g. the understanding of a defined system, practice, method or procedure).

A knowledge and understanding of health and social care policies and procedures, as well as keeping informed on data protection, equal opportunities and antidiscrimination laws.

Knowledge of the range of services provided by Health and Community Services by other States departments and by the private and voluntary sectors.

Technical / Work-based Skills

A full driving license and ability to attain an Assistant Social Workers Senior Social Workers LTC Nurses Social Workers Adult Social Care Team Manager This relates to the skills specific to the job, e.g. language fluency, vehicle license etc. enhanced DBS clearance check.

This relates to more general characteristics required to do the job effectively, e.g. effective written communication skills, ability to delegate, motivation or commitment etc.

High quality communication and people skills, including previous interactions with members of the public, health professionals, care agencies.

Have the ability to autonomously work with capacity for independent and responsible decision making with the ability to recognize and seek support of a qualified professional as appropriate.

Good IT and computer skills.

The ability to be flexible, precise, organized, and professional whilst working in pressurized, and challenging environments, and situations.

Manage and maintain a caseload of clients and manage working time appropriately and safely. Sound planning and organisational skills are an essential component of the job.

Experience

 This is the proven record of experience and achievement in a field, profession or specialism.

This could include a minimum period of experience in a defined area of work if required by an external body (for example a period of post/qualification experience).

Experience in a social care setting and/or working closely with the public is essential. Evidence of previous work or experience within a customer facing service environment.

Criteria relating to Safeguarding Other requirements needed to confirm suitability to work with vulnerable people e.g. attitudes, skills, experience etc.

Experience in risk assessments and safeguarding vulnerable people is required together with the ability to record accurately, objectively and within timescales.
