We are seeking a compassionate and motivated Assistant Social Worker to join our team. In this role, you will support the Adult Social Care Team in delivering excellent care and services to our clients. Your responsibilities will include assisting with case management, conducting client assessments, completing care plans and conducting reviews. Your work will ensure our clients receive the necessary support to enhance their quality of life, whilst maintaining their independence for as long as possible.

To support and assist the Adult Social Care team in the provision of a comprehensive social care assessments, early intervention and reviews for the vulnerable adult population in Jersey alongside their families and carers. To support with delivering the day to day services, accessing and facilitating resources to manage risk and meet identified needs.

The main duties of an Assistant Social Worker are:

To holistically assess the social care needs of a service user, to include early intervention community support, providing ongoing care planning and review services proportionate to the role as agreed by their line manager. This may involve working alongside other health professionals and departments to ensure positive and effective MDT working practices are followed.

Deliver appropriate, open, accurate and straightforward communication to service users, their families and carers, clearly explaining the responsibilities of the social work team and subsequent services that could support the individual. Where possible promote independence and provide relevant information to service users to enable them to make safe and informed decisions regarding their care needs and daily routines.

This may require the Assistant Social Worker to advocate on behalf of a client, supporting them to appropriate meetings or visits in line with their care plan objectives.

To assess and identify appropriate funding and support to access community resources to meet the identified needs of the service user e.g. Long Term Care benefits, income support and charity funding.

The Assistant Social Worker is required to manage their own caseloads in consultation with their line manager and is required to work with a degree of autonomy, prioritize cases, manage their time effectively, and make appropriate recommendations in the interests of the service users’ health and well-being.

Maintain the highest standard of personal and professional conduct, adhere to relevant codes of conduct, policies, procedures and legislation and take responsibility for maintaining and improving knowledge and skills comparable to the role. Assume responsibility for the planning and progression of their training and development. This includes attending relevant training courses and presentations, and researching into areas of social care learning.

Create and maintain clear and accurate records to ensure that all relevant information is held as required by established procedures and best practice and in line with data protection, confidentiality, disclosure and consent policies, always bearing in mind that such documents may be disclosable.
