Job Overview
We are seeking a CAMHS Transition Support Worker (CS08) to join our team, supporting young people as they move from Child and Adolescent Mental Health Services into adult or community services. This is a vital role focused on improving continuity of care, reducing risk and ensuring young people and families feel supported during this critical stage. You will work closely with the multidisciplinary team and partner agencies to coordinate transition plans, provide practical and emotional support, and promote engagement and positive outcomes.
This role is ideal for a compassionate and motivated individual with experience working with children, young people or families in a health or social care setting. You will bring strong communication and organisational skills, an understanding of mental health and child development, and a commitment to improving outcomes. In return, you will be part of a supportive team and contribute to delivering a key priority within the Children’s Health and Wellbeing Strategy - ensuring safe, effective and person-centred transitions into adulthood.
