Job Description

Job Purpose

The Community Therapy Support Worker provides support across community and hospital settings, working independently in clients’ homes and local environments. The role involves assessing individual needs, supplying appropriate disability loan equipment, and offering recommendations for straightforward home adaptations

Statutory Responsibilities

Statutory responsibilities: Active engagement, participation and compliance with any other statutory responsibilities applicable to the role, as amended from time to time

Job Specific Outcomes 1 of 2

Participate in the screening, assessment and intervention of patients/clients referred to the service and undertaking the work unsupervised both on hospital sites and in the patient’s own home.

 Manage a case load of patients/clients in the community delegated by the Occupational Therapist, and co-ordinating and carrying out any intervention necessary.

Evaluate the needs of the patients/clients and plan long/short term strategies to meet the identified needs and promote maximum independence.

Promote the active involvement of patients/clients in decision making processes involving their personal independence and disability management.

Establish and maintain effective working relationships with family members, carers and staff from other departments/agencies involved with the patients/clients in order to provide intervention and on-going support in the community, referring onwards to other professions where necessary.

Attend relevant staff and clinical meetings to discuss and agree case management and reporting on client progress, including the provision of written reports as necessary.

Be responsible for creating and maintaining appropriate patient/client records to monitor effectiveness and facilitate intervention planning and ensuring that client confidentiality requirements are met.

Attend relevant education and training in order to maintain Continuous Professional Development in line with Performance, Review and Appraisal.

Assist in ensuring the maintenance of equipment and resources in accordance with Health and Safety policies.

Qualifications

Essential:Educated to A Level standard.

Desirable: NVQ Level 3 qualification in a Health and Social Care field 

Knowledge

Knowledge and understanding of Health and Social Services departments

Knowledge of the wide range of disability equipment and its uses to promote personal independence for the client and carer. 

Technical Work Based Skills

Clean driving licence.

General skills / attributes

Good standard of numeracy and literacy together with excellent skills with Windows based software.

Good interpersonal skills and have an interest in promoting the personal independence of individuals.

Physically fit due to community-based working and the installation of community equipment. 

Experience

Experience of team working and networking with other agencies and departments
