Job Summary

Here in Jersey, we have our own Health Care system, separate from the NHS. We are looking to develop and improve our healthcare services towards a modern, fully digitised and future-proof system, in advance of our new hospital (planned to open in 2029). Health & Care Jersey is seeking to appoint an enthusiastic and dynamic substantive Acute and General Medicine Consultant to join our already passionate consultant body.

The Post

Health & Care Jersey is seeking to appoint an enthusiastic and dynamic substantive Consultant Physician in General Medicine to join an enthusiastic consultant body managing general internal medicine.

 

The primary commitment is to general medical services but there are opportunities for suitably qualified candidates to have a special interest in one of Respiratory, Renal Medicine, Acute Medicine, Frailty and Stroke.

Duties

• Lead the care of inpatients on a General Internal Medicine/ Specialty ward.

• Work within and strive to improve standards, procedures and systems to ensure safe, high quality and efficient patient care from the point of entry to admission or discharge

• Work within the multidisciplinary team to ensure effective team working in the provision of acute medical care to patients on a day-to-day basis.

• Practice within and participate in developing clinical guidelines to ensure best practice for the management of wide range medical conditions.

• Participate in clinical governance including audit and quality assurance processes, incident investigation and quality improvement, risk management.

• Work collaboratively within the department, the emergency and acute medical teams, other in-patient teams and primary and community services to maximize benefits to patient care.

• Teach and train junior doctors and students.

• Become a qualified educational supervisor and work as Clinical and/or Educational Supervisor to a number of trainees dependant on job plan.

• To teach and train staff in other medical disciplines, nursing and support staff as appropriate.

• Take part in major incident planning.

• Take part in the administration of the department and the involvement in the management of resources will be regarded as important.

• To promote General medicine as a Specialty both within the organisation and externally.

• To deliver personalised, evidence informed medical care plans to all patients that are appropriately documented and regularly evaluated.

• To participate and attend relevant services’ MDTs.

• To liaise with tertiary referral centres as appropriate to provide patient centred care & service improvements.

• Maintain good quality up to date medical records including timely discharge letters and summaries.

• To ensure appropriate documentation and coding is used to provide consistent audit and monitoring of the service.

• To adhere to local prescribing guidelines.

• The post holder should be adaptable, flexible and innovative in order to meet the ongoing challenges within the service. The responsibilities of the role may change from time to time.

• To comply with infection prevention & control requirements.

 

Leadership and Management

• Take part in management and audit activities to support the Care Group Leadership team & Public Health in planning for services and ensuring the service meets key performance indications including excellent patient care.

• The post holder will be expected to take part in management and audit activities and represent the service at Hospital meetings, as agreed with colleagues.

Teaching and Research

The department is recognised for Foundation Doctors from the Wessex deanery and medical students from Southampton and active teaching is encouraged in this role. It is expected that the post holder will participate in the training of medical staff and ensure that the training and learning needs are identified and facilitated. This includes educational support for the clinical fellows recruited directly to Health & Care Jersey. The post holder should deliver training to a range of professionals across the multi-disciplinary team. Opportunities for participating in research are supported when appropriate. The Health & Care Jersey has a policy for professional leave and is supportive of external duties for the good of the wider profession.

On-Call Rota

The post holder will be expected to participate in the General medical on call rota. This is currently 1:8 including weekends and public holidays. The on-call consultant is required to do a ward round on Saturday, Sunday and Public Holidays. The frequency of call out from home is very low (less than 1 per month).

The Specialty

There will be an opportunity to develop your specialist interest in accordance with the medical care group plans. Close working with the relevant tertiary centres will be supported.

Medical Services Care Group

 

Medicine in Jersey is fully age and specialty integrated. There are 3 medical wards, Plemont, Rozel and Corbiere. A 5-bedded Enhanced Care Area is sited on AAU and there is a separate intensive care unit led by intensivists. There is also an Ambulatory Emergency Care Unit led by the Acute Medicine Department. There is a 25 bed Acute Assessment Unit.

 

SAS doctors are responsible for the day-to-day management of their patients on the wards with the help of clinical fellows, FY1 or FY2 doctor whom they closely supervise.

 

On call, the SAS doctor is responsible for seeing patients referred by the Emergency Department and reviewing patients admitted to the AAU by GPs in conjunction with the Acute Medicine team. A core responsibility of the Medicine Care Group is the provision of a safe acute medical service. The Care Group always provides named Consultant supervision for non-Consultants and ensures that prompt review and treatment is provided for acute medical emergencies referred by General Practitioners or the Emergency Department.

 

Consultants are responsible for producing and updating guidelines for the management of acute medical emergencies in their specialist area.

In addition to outpatient clinics and 3 general medical wards containing 63 beds, the Medical Service Care Group is involved in the management of several specialist facilities. These include:

A well-equipped Clinical Investigations Department providing echocardiography, exercise ECG, respiratory assessment and GI physiology.

An endoscopy suite providing a full range of endoscopy and bronchoscopy services.

Diabetes Centre providing outpatient care for the diabetic population of Jersey, including retinal photography and specialist nursing support.

Renal/Dialysis unit, providing emergency, acute and longer-term renal support as well as management of transplanted patients and assessment and treatment of renal conditions.

Proposed Job Plan

All consultants have a standard 10 PA contract for public work. Within this, all consultants have 1.5 core SPA for CPD activities in line with AoMRC guidance. Most consultants will have a further 1 SPA for agreed activities which support HCJ.

 

The final job plan will be agreed with the Clinical Lead for General & Specialist Medicine, the Chief of Service for Medical Services, Hospital Director and ratified by the Medical Director. The post holder’s job plan will be reviewed annually and will be appraised according to the current guidance.

 

Provision will be made for a lunch break between morning and afternoon sessions if both are clinical.

 

Please note within 3 months of commencement in post, you are expected to agree with your clinical manager a prospective job plan.

 

*TOIL is granted for on call commitments at 1PA per on call. **SPA is guaranteed as a minimum of 1.5 SPA of which must be done within the hospital and 0.50 SPA to be worked flexibly. Additional Programmed Activities (SPAs) are available for education and clinical supervision, lead roles and extra capacity (DCCS). Private Practice can be agreed as part of the job plan process and must be recorded. The final job plan will be agreed with the Clinical Director and Chief of Service, before final approval with the Medical Director. The post holder’s job plan will be reviewed annually and will be appraised according to the current guidance. In the event of inability to agree a job plan either at appointment or upon annual review, or in the event of issues arising from the appraisal process, negotiation will be with the Chief of Service.

Qualifications and Skills

Essential: 

Primary Medical Degree

Higher Specialty qualification: MRCP or equivalent

Full Registration with the General Medical Council (GMC) with a Licence to practise (for overseas candidates it is mandatory that a licence is obtained prior to appointment)
