Health and Care Jersey is seeking to appoint an enthusiastic and dynamic substantive Consultant in General Medicine with an Interest in Care of the Elderly to join our already passionate consultant body.

The Post

This post is an exciting opportunity to join the newly formed Elderly Care & Frailty Department. The role will mainly focus on the development of frailty services for the island of Jersey as well as having ward commitments for both general internal medicine. The role will lead on the development of the frailty unit interfacing secondary care with primary care. The role will have a rotational commitment for rehabilitation which is situated on-site at Jersey General Hospital as well as outpatient clinical sessions.

Duties

1. Contribute to development of the Care of the elderly service in Jersey build and develop a frailty service which ensures integration between primary and secondary care.

 

2. Provide with the other Consultants a system of mutual out-of-hours cover for General Internal Medicine and cover for annual, study and professional leave.

 

3. Provide a consultation and advisory service to medical colleagues in other specialities in Health and Care Jersey.

 

4. Liaise with medical staff in other specialities and participate in the regular clinical meetings and other post-graduate activities of the hospital.

 

5. Take part in the continuing medical education programme. Health and Care Jersey will provide study leave expenses in line with national terms and conditions of service.

 

6. To undertake outpatient clinics for both frailty & elderly care.

 

7. To set-up, lead, participate and attend relevant services’ MDTs.

 

8. To liaise with tertiary referral centres as appropriate to provide patient centred care & service improvements.

On Call Rota

The appointee will take part in the General Medicine on-call rota of one in eight. During the on-call the consultant will cover all medical inpatients in the event deterioration or medical emergencies. For weekday on-call a post take ward round is undertaken at 5pm, the following morning the on-call consultant is only required to post-take patients who are not on the Emergency Assessment Unit.

 

During weekends and bank holidays the consultant will undertake post take ward rounds and discharge ward rounds in line the medical model at 8am & 5pm.


Weekday on-calls operate 8am-8am the following day. Weekend on-calls operate from 8am on Friday to 8am Monday.

The Specialty

In patient care is based on medical wards in Jersey General Hospital. The post holder will be expected to deliver Consultant led care on that ward focussing on Elderly Care. There are currently no specialty outpatient clinics, and it is an opportunity for the newly appointed Consultant to develop a speciality outpatient service in discussion with the Care Group Chief of Service.

Medical Services Care Group

 

Medicine in Jersey is fully age and specialty integrated. There are 4 medical wards, Bartlett, Plemont, Rozel and Corbiere.


A 5-bedded Enhanced Care Area is sited on AAU and there is a separate intensive care unit led by intensivists. There is also an Ambulatory Emergency Care Unit led by the Acute Medicine Department. There is a 14 bed Acute Assessment Unit.

 

SAS doctors are responsible for the day-to-day management of their patients on the wards with the help of clinical fellows, FY1 or FY2 doctor whom they closely supervise.

 

On call, the SAS doctor is responsible for seeing patients referred by the Emergency Department and reviewing patients admitted to the AAU by GPs in conjunction with the Acute Medicine team. A core responsibility of the Medicine Care Group is the provision of a safe acute medical service. The Care Group always provides named Consultant supervision for non-Consultants and ensures that prompt review and treatment is provided for acute medical emergencies referred by General Practitioners or the Emergency Department.

 

Consultants are responsible for producing and updating guidelines for the management of acute medical emergencies in their specialist area.

The post holder will share responsibility for the safeguarding and welfare of all children, adults and/or families within their care and should be familiar with the signs and symptoms of abuse and know what to do if any such concerns are raised. The post holder requires to be aware of, and comply with, all child protection, safeguarding policies and procedures.


As this position will involve working in ‘regulated activity’, the successful applicant will be subject to an Enhanced DBS disclosure as part of their pre-employment clearances. Any offer of employment is subject to satisfactory clearances being obtained.

Teaching

The Hospital has been a recognised ATLS centre since 1990 and ALS centre since 1995. Four courses of each are held annually, one of the ATLS courses is held in conjunction with an ATNC course. APLS courses commenced in 2002, and it is intended to run two courses each year. The hospital has a Governance committee supported by a team of clinical audit officers. The post holder will be expected to contribute and participate in the Department’s ongoing clinical audit programme. Health and Care Jersey work with Health Education Wessex to provide high quality medical education to FY1, FY2 and GP Trainee doctors on a rotational basis.


Medical Students also rotate through Medicine and Surgery from Southampton. The post holder will assist junior staff to identify and meet learning objectives by using the principles of teaching and learning.

Continued Professional Development

The post holder will seek opportunities to develop knowledge, skills and experience and be expected to maintain continued professional development to satisfy the requirement of annual appraisals including attendance at relevant courses and conferences, involvement at locally organised CME events and comparative audit. Health and Care Jersey supports the requirements for Continuing Medical Education and is committed to providing reasonable amounts of time and financial support for these activities. Study leaves of 30 days in 3 years with a generous annual study leave allowance is provided, subject to the approval of the Care Group Head / Medical Director. The post holder will be expected to meet the Continuing Professional Development requirements of their relevant Royal College as required for revalidation.

Appraisal and Review

The post holder is required to participate in the annual appraisal and review process which has been implemented for all consultants and is in line with the GMC guidelines for revalidation. Training is available for appraisers.

Registration

It is a legal requirement for all doctors to be on the General Medical Council’s Specialist Register before being able to take up a consultant post. Specialty trainees are able to apply for a consultant appointment provided the expected date of the award of their Certificate of Completion of Training (CCT) (or recognised equivalent if outside the UK) falls no more than 6 months after the interview date for the Consultant position and candidates will be asked to provide proof of this prior to shortlisting.

Proposed Job Plan

All consultants have a standard 10 PA contract for public work. Within this, all consultants have 1.5 core SPA for CPD activities in line with AoMRC guidance. Most consultants will have a further 1 SPA for agreed activities which support Health and Care Jersey.

 

The final job plan will be agreed with the Clinical Lead for General Medicine, the Chief of Service for Medical Services, Hospital Director and ratified by the Medical Director. The post holder’s job plan will be reviewed annually and will be appraised according to the current guidance.

 

The Job Plan can be discussed at the time of employment but will broadly be based on the provisional timetable outlined below until the joint review takes place between the successfully candidate and other consultant pathologists. The Job Plan will be authorised by the Chief of Service and is subject to annual review. However, it should be noted that this service requires a flexible approach.

 

The final job plan will be agreed with the Clinical Director and Chief of Service, before final approval with the Medical Director. The post holder’s job plan will be reviewed annually and will be appraised according to the current guidance.

 

In the event of inability to agree a job plan either at appointment or upon annual review, or in the event of issues arising from the appraisal process, negotiation will be with the Chief of Service.

Qualifications and Skills

Essential: 

Primary Medical Degree

Full Registration and specialist registration with the General Medical Council (GMC) with a current License to practice (for overseas candidates it is mandatory that a license is obtained prior to appointment).

Higher Specialty qualification: MRCP & FRCPath or equivalent Qualifications for Specialist Registration.

Applicants that are UK trained, must ALSO be a holder of a Certificate of Completion of Training (CCT), or be within six months of award of CCT by date of interview.

Applicants that are non-UK trained, will be required to show evidence of equivalence to the UK CCT.

Inclusion on the GMC Specialist Register/Certificate of Completion of Training (CCT) or equivalent in the relevant Specialty (or be within 6 months of obtaining this at the date of the interview - documentary evidence of this must be provided to support your application) or equivalent.

ALS/ATLS trained.

 

Desirable: 

Relevant experience at a consultant level in a similar setting.

Higher Degree MSc, MD or PhD or equivalent.
