The Post

Health and Care Jersey (HCJ) is looking to appoint an enthusiastic and dynamic substantive Consultant Haematologist to join our dedicated team delivering care across our Haematology, blood transfusion, and donor service. 

 

Working in partnership with their colleague, the successful candidate will provide a comprehensive Consultant-led Haematology service for inpatient and outpatient services and appropriate support to facilitate successful Blood Sciences laboratory and Donor Services.

 

Opportunities exist for suitably qualified candidates that have special interests in blood transfusion, donor, lymphoma, myeloma, and haemophilia services for the island.

Clinical Duties

- Further develop a service of excellence for haematology & blood transfusion which ensures a seamless integration between primary & secondary care aligned to the New Healthcare Facilities Programme.

- Lead the interface with Primary Care.

- Provide full continuing clinical responsibility for patients under the care of the Clinical Haematology team, and for the provision of treatment relevant to the blood condition whilst in hospital.

- To provide care for patients with general and malignant haematology patients including contribution to existing lymphoma clinic cover.

- Provision of advice and assistance to consultants in other specialties, and the proper delegation to and training of junior medical staff.

- Participate in a Consultant rota for laboratory supervision and clinical liaison and to support to the Clinical Haematology service on an inpatient, day care and outpatient basis.

- The appointee will contribute to the clinical management of Haemato-Oncology patients at the weekly MDT meeting.

- The post-holder will participate in consultant meetings, ward rounds and Haematology team meetings.

- The appointee will be required to undertake appropriate personal development and annual medical appraisal in order to meet the requirements of medical revalidation.

- Appropriate agreed study leave is available with timing agreed in consultation with the other Consultant Haematologist. Time and facilities for audit are available as identified within the job plan. The timing of annual leave will be pre-agreed with the other Consultant Haematologist and then endorsed by the relevant Clinical Director.

- The appointee will be expected to contribute to establishing policies, procedures, audit and clinical research activities of the Haematology Service. They will predominantly be involvement National Cancer Research Network (NCRN)-badged multicentre clinical trials.

- The appointee may be required to undertake other duties appropriate for a Consultant Haematologist not otherwise stated.

- The appointee can undertake private practice at Jersey General Hospital.

- To provide clinical advice & support to primary care colleagues and other consultants within Health & Care Jersey medical model.

- To deliver personalised, evidence informed medical care plans to all patients that are appropriately documented and regularly evaluated.

- To provide advice and timely medical reviews and interventions as part of the interdisciplinary model of care.

- Maintain good quality up to date medical records including timely discharge letters and summaries.

- To ensure appropriate documentation and coding is used to provide consistent audit and monitoring of the service.

- To adhere to local prescribing guidelines.

Teaching and Research

The department is recognised for junior doctors from the Wessex deanery and medical students from Southampton and active teaching is encouraged in this role. It is expected that the post holder is participating in the training of medical staff and that the candidate will ensure that the training and learning needs are identified and facilitated. This includes educational support for the clinical fellows recruited directly to Health & Care Jersey.

 

Opportunities for participating in research are supported when appropriate. The Health & Care Jersey has a policy for professional leave and is supportive of external duties for the good of the wider profession.

On Call Rota

The role does not currently have on-call service, the service Is provided by Bournemouth Royal Hospital NHS Trust (RBH).

The Specialty

Jersey General Hospital (JGH) currently offers a wide range of clinical and laboratory haematology services. Adult Haematogical Oncology is delivered to BCSH Level 2a as defined by the BCSH Guideline (2010). Patients requiring BCSH Level 2b care and above including the management of acute leukaemia, autologous stem cell transplantation and plasma exchange therapy are transferred electively or as emergencies to Royal Bournemouth Hospital.

 

There are strong links with University Hospitals Southampton (UHS) for clinically aggressive lymphoma, allogeneic stem cell transplant and radiotherapy. A lymphoma consultant from UHS is contributing to the lymphoma service on a fortnightly basis. UHS also hosts the regional Teenage and Young Adult (TYA) Haematology-Oncology service. There are also links with RBH for Leukaemia induction and King’s Partners, London for specialist tertiary referrals.

 

There are occasional referrals made to other UK hospitals, including GOSH for paediatric malignancy. This is approached on a shared-care basis between the Jersey Paediatric Team and GOSH. The Jersey Haematology Service provides laboratory diagnostic support for paediatric blood cancers, however, shares clinical care of TYA patients with the UHS TYA Unit on a case-by-case basis.

 

The Haematology Department is undergoing significant restructuring, and the successful appointee will be expected to contribute to shaping developments. There are opportunities for further service expansion with the potential to develop academic activities and strengthen external links.

Multi-Disciplinary Blood Cancer Team Meetings (MDTMs)

A Jersey-Guernsey local Haem-Oncology MDTM is held on site every Tuesday morning. The UHS Lymphoma Network MDTM occurs in Southampton every Tuesday morning The Dorset Cancer Network Blood Cancers MDTM operates each Friday morning including our Bournemouth referral partners. All MDT’s are attended via video link.

 

Clinical Facilities – Outpatient & Day-Care

A day care Chemotherapy Unit on Le Quesne Unit is shared with Medical Oncology. Le Quesne Unit also hosts haematological oncology outpatient clinics including lymphoma clinic. A separate Medical Day Care Unit hosts an infusion/transfusion service. General Haematology with some haem-Onc Clinics currently held in the Medical Outpatient area.

The Specialty

Clinical Facilities – Inpatients

Inpatients with blood disorders are managed on general medical wards and supervised hour-to-hour by junior medical staff working in teams led by Physicians-with-an-Interest. Shared care local Specialist Haematology input runs in office hours. Out-of-hours Consultant Haematologist cover is by telephone support from colleagues at Royal Bournemouth Hospital (RBH) well established and any patient admitted who cannot reasonably be managed at night/weekends by a general medical junior doctor team are transferred to specialist Haematology beds on ward 11 at RBH. 

The appointee will have admitting rights for public patients via the Emergency Assessment Unit (EAU).

The appointee will have admitting rights for the sole-care private patients to the Private (Sorel) Ward by booking a bed with the Ward Staff. Private Haematology outpatients are currently seen in the Pathology Laboratory Clinical Room, as are Private patients of other teams requiring blood sampling. However, any private practice must be undertaken outside of contractual obligations. Consultants will be billed for the use of clinic rooms for private patients.

 

Haematology & Transfusion Laboratory Service

The Haematology & Transfusion Laboratory function is currently embedded in a Blood Sciences Department and provides a routine service to our users from 0800 hours to 1800 hours with a structured out of hours service at other times, provided by a multidisciplinary out-of-hours team. The service provides a maximum 12-hour turn-around time for all routine work undertaken on Island and a one hour turn around for all emergency work. UK reference centres are used for analyses that are not provided currently in Jersey including Flow-cytometry, Cytogenetics, Molecular Genetic studies and the assessment of abnormal haemoglobins. The Laboratory Service hosts and supports and independent Blood Donor Service that routinely allows Jersey to be 95-96% self-sufficient in red cell provision. The Island also benefits from an unusually high repeat donation rate.

Laboratory Information System (LIS)

Laboratory analysers feed into a range of middleware packages that interface with an OMNILAB LIS database as the sole source of Jersey laboratory data. The dataset dates back continuously to May 1999. The current OMNILAB version is V11.

The Specialty

Antenatal Red Cell Serology

There are 1000-1100 births annually in Jersey. With a significant blood transfusion scientific expertise on-Island to support an independent Blood Donor Service the bulk of antenatal red cell serology is managed locally, except for anti-c and anti-D samples requiring auto-analyser quantitation. The Island has not implemented a program of Routine Antenatal Anti-D Prophylaxis (RAADP) due in part to resourcing and in part to an unusually low Anti-D sensitisation rate, with active and timely post-event prophylaxis in a small community. The introduction of routine fetal D-typing is however being actively pursued to permit a target program of RAADP.

 

Haemoglobinopathy Screening

Jersey is intermediate risk in terms of antenatal haemoglobinopathy screening, including the indigenous Jersey population descended from areas of Northern France and a large Portuguese/Madeiran population both a significant risk of beta-thalassemia. The screening currently undertaken is: 

Antenatal haemoglobinopathy screens – selective testing guided by midwife questionnaire Neonatal haemoglobinopathy screens – universal; based on Guthrie-type blood spot.

 

The Jersey Blood Sciences Service no longer offers in-house Haemoglobinopathy testing adult samples for Haemoglobin electrophoresis, HbA2 and HbF are outsourced at present to Birmingham HEFT. Guthrie neonatal cards are processed via Portsmouth. The appointee would have an opportunity to develop the service to meet UK guidelines.

 

Jersey does not employ a Haemoglobinopathy Counsellor. Neonatal screen positives are managed by Paediatric staff.

 

Blood Pack Production and Jersey Donor Service

Jersey organises its own non-NHS independent Blood Transfusion Service.

 

The Service calls and medically screens prospective Island blood donors and aims to manufacture 2800 plasma-depleted & white cell depleted red cell concentrates per year.

 

The Jersey Donor Service is licensed as a producer of red cell concentrates under the Medicines (Jersey) Law 1995 and the MHRA inspect in an advisory capacity in July 2018 and recommended re-licensing of the service subject to recalibration of the on-site blood irradiator.

 

The Donor Service regularly achieves 95-96% Island self-sufficiency in red cell components; however, Jersey depends on UK NHSBT supply of platelet, FFP and cryoprecipitate packs. These are imported by air according to clinical need and where weather permits scheduled flights. In poor weather and dire emergency H.M. Coastguard helicopters may assist.

Therapeutic Venesection Service

There are a substantial number of Haemochromatosis patients in Jersey as a result of active case finding, together with a significant number of Myeloproliferative Neoplasm (MPN) patients requiring therapeutic venesection. Of the overall 1.66 FTE Donor Nurse, 0.4FTE is currently committed to therapeutic venesection of patients with Haemochromatosis and Myeloproliferative Neoplasms.

 

Nurse-led Anticoagulant Dosing Service

Since 1997 Jersey Hospital has offered a free of charge nurse-led Coumarin anticoagulant dosing service alongside the Primary Care provision in the Island which runs on a fee-for-service pre-NHS model. The advent of the new generation of Directly Acting Oral Anticoagulants (DOACs) has resulted in a significant decline in Warfarin use and need for INR dose monitoring. There are number of complex patients managed by a part time anticoagulant nurse.

The Specialty

Anticoagulant Dosing Activity

With an ageing population and revisions to UK Atrial Fibrillation guidelines the Anticoagulant dosing workload rose to a peak of 12,000 attendances pa. The year-on-year Anticoagulant dosing clinic activity is summarised in the Table. Following the progressive introduction of DOAC drug therapies as an alternative to Warfarin annual anticoagulant dosing attendances have steadily fallen to fewer than 5000 per year now.

 

Haemophilia Service

There are 110-120 bleeding disorder patients registered in Jersey the bulk of whom have been managed on-Island for the past 25 years. Three children with more severe forms of haemophilia are on regular prophylaxis and are managed by the Jersey paediatric team on a shared care basis with the GOSH Haemophilia Centre. Similarly, three adults on regular prophylaxis are shared with the Royal Free Hospital (RFH) Haemophilia Centre. 

 

Otherwise, post-trauma haemophilia care is delivered locally as is the management of modest degrees of elective surgery including invasive dental surgery. More complex elective surgery in more severe bleeding disorders is referred to a UK Haemophilia Comprehensive Care Centre, usually RFH.

 

As an isolated community with a significant number of bleeding disorder patients the Hospital routinely stocks Recombinant Coagulation Factors 8 & 9, Plasma-derived Factor 7, Beriplex 4-component prothrombin complex, a plasma-derived Von Willebrand Concentrate and Recombinant Factor 7a.

 

Hospital Transfusion Committee (HTC)

The HTC is operational, led by acute medicine consultant. The HTC aims to meet 4-monthly and reports to the Quality Assurance Committee. There is no Hospital transfusion team in place due to lack of a Transfusion Practitioner Nurse. 

 

The Donor Service is MHRA accredited since 2018. The Laboratory participate fully in SHOT, SABRE reporting and is involved in selected National and Regional Blood Transfusion Audits.

 

The Blood Transfusion Laboratory is fully automated and at present electronic blood issue has not been implemented as the bulk of blood issued in Jersey is home produced and has not been subject to independent duplicate ABO/D typing as part of the production process. A Maximum Blood Order Schedule (MSBOS) is in place.

 

Office Space Dedicated office space and secretarial time will be provided as part of the Hospital investment in this appointment.

Medical Services Care Group

Medicine in Jersey is fully age and specialty integrated. There are 3 medical wards, Plemont, Rozel and Corbiere. A 5-bedded Enhanced Care Area is sited on AAU and there is a separate intensive care unit led by intensivists. There is also an Ambulatory Emergency Care Unit led by the Acute Medicine Department. There is a 25 bed Acute Assessment Unit.

 

SAS doctors are responsible for the day-to-day management of their patients on the wards with the help of clinical fellows, FY1 or FY2 doctor whom they closely supervise.

 

On call, the SAS doctor is responsible for seeing patients referred by the Emergency Department and reviewing patients admitted to the AAU by GPs in conjunction with the Acute Medicine team. A core responsibility of the Medicine Care Group is the provision of a safe acute medical service. The Care Group always provides named Consultant supervision for non-Consultants and ensures that prompt review and treatment is provided for acute medical emergencies referred by General Practitioners or the Emergency Department.

 

Consultants are responsible for producing and updating guidelines for the management of acute medical emergencies in their specialist area.

All consultants have a standard 10 PA contract for public work. Within this, all consultants have 1.5 core SPA for CPD activities in line with AoMRC guidance. Most consultants will have a further 1 SPA for agreed activities which support HCJ.

 

The final job plan will be agreed with the Clinical Lead for Pathology, the Chief of Service for Medical Services, Hospital Director and ratified by the Medical Director. The post holder’s job plan will be reviewed annually and will be appraised according to the current guidance.

The Job Plan can be discussed at the time of employment but will broadly be based on the provisional timetable outlined below until the joint review takes place between the successfully candidate and other consultant pathologists. The Job Plan will be authorised by the Chief of Service and is subject to annual review. However, it should be noted that this service requires a flexible approach.

 

Timetable – specific roles and timings to be agreed by the successful candidate and the current substantive colleague in conjunction with the Clinical Lead in Pathology.

The final job plan will be agreed with the Clinical Director and Chief of Service, before final approval with the Medical Director. The post holder’s job plan will be reviewed annually and will be appraised according to the current guidance. In the event of inability to agree a job plan either at appointment or upon annual review, or in the event of issues arising from the appraisal process, negotiation will be with the Chief of Service.

Qualifications and Skills

Essential:

Primary Medical Degree

Full Registration and specialist registration with the General Medical Council (GMC) with a current License to practice (for overseas candidates it is mandatory that a license is obtained prior to appointment).

Higher Specialty qualification: MRCP & FRCPath or equivalent Qualifications for Specialist Registration

Applicants that are UK trained, must ALSO be a holder of a Certificate of Completion of Training (CCT), or be within six months of award of CCT by date of interview.

Applicants that are non-UK trained, will be required to show evidence of equivalence to the UK CCT

Inclusion on the GMC Specialist Register/Certificate of Completion of Training (CCT) or equivalent in the relevant Specialty (or be within 6 months of obtaining this at the date of the interview - documentary evidence of this must be provided to support your application) or equivalent



Desirable: 

Relevant experience at a consultant level in a similar setting

Higher Degree MSc, MD or PhD or equivalent

ALS/ATLS trained

Leadership skills in managing change in haematology services

General Experience

Essential:

Experience in haematology medicine

Evidence of successful interface between secondary and primary care

Enthusiastic Teacher

 

Desirable: 

Management Responsibility / Leadership Experience

Specialist Experience

Essential: 

Clinical management of haematology patients including general and malignant haematology.

Experience in blood transfusion

 

Desirable: 

Experience within a blood donor & manufacturing service

Specialist interest (E.g transfusion, laboratory haematology)
