Health and Care Jersey is seeking to appoint an enthusiastic and dynamic substantive Consultant Nephrologist to join our already passionate consultant body.

The Post

Health and Care Jersey is seeking to appoint an enthusiastic and dynamic substantive Consultant in Renal Medicine. The successful application will be the lead for Renal Medicine, supported by a middle grade and a team of specialist nurses to provide a comprehensive service in renal medicine. The successful applicant must be dual accredited in nephrology and GIM.


The successful candidate will have broad experience of inpatient and outpatient in all aspect of nephrological practice and fully competent in diagnostics and therapeutics. The applicant will undertake Inpatient specialist reviews of patients under the care of other disciplines.


For newly appointed consultants a mentor will be allocated to support their induction to the post.

There are dedicated medical secretaries supporting the Consultant and their team. The Consultant will have a shared office space with IT facilities shared across Consultants from other specialities.

Duties

• Continue to jointly develop a service of excellence for Renal Medicine which ensures a seamless integration between Jersey and tertiary centres in England.
• Participate in the ongoing development of a Renal strategy for the island of Jersey.
• To work in close partnership and liaison with Primary Care and other specialist services.
• To undertake inpatient specialist reviews for Renal Medicine.
• To undertake outpatient clinics for Renal Medicine.
• Provide diagnostic and appropriate interventional services.
• To provide clinical advice & support to primary care colleagues and other Consultants within Health and Care Jersey medical model.

• To deliver personalised, evidence informed medical care plans to all patients that are appropriately documented and regularly evaluated.
• To provide advice and timely medical reviews and interventions as part of the interdisciplinary model of care.
• To participate and attend relevant services’ MDTs.
• To liaise with tertiary referral centres as appropriate to provide patient centered care & service improvements.
• Maintain good quality up to date medical records including timely discharge letters and summaries.
• To ensure appropriate documentation and coding is used to provide consistent audit and monitoring of the service.
• To adhere to local prescribing guidelines.
• The post holder should be adaptable, flexible and innovative to meet the ongoing challenges within the service. The responsibilities of the role may change from time to time.
• To encourage dialogue with primary care clinicians to for admission avoidance and advice.

 

Leadership and Management

• Take part in management and audit activities to support the Care Group Leadership team & Public Health in planning for services and ensuring the service meets key performance indicators including excellent patient care.

• The post holder will be expected to take part in management and audit activities and represent the service at Hospital meetings, as agreed with colleagues.

• The post holder will be expected to be fully involved in a ‘Risk Management” programe and support the Governance agenda.

• Represent the service at hospital meetings, as agreed with colleagues.

 

Teaching and Research

It is expected that the post holder is participating in the training of medical staff and that the candidate will ensure that the training and learning needs are identified and facilitated. This includes educational support for the clinical fellows recruited directly to Health and Care Jersey (HCJ).


The post holder should deliver training to a range of professionals across the multi-disciplinary team in renal medicine to create a service of excellence for Jersey.


Opportunities for participating in research are supported when appropriate. HCJ has a policy for professional leave and is supportive of external duties for the good of the wider profession.

The Specialty

Jersey General Hospital has 219 beds including an intensive care unit,. Renal medicine provides a comprehensive service for all aspects of kidney disease for adult patients with tertiary level support from Portsmouth with vascular access surgeries and transplant surgery supported by Cambridge University Hospital. Paediatric renal care delivered by University Hospital Southampton (UHS).


The renal unit currently manages 35 haemodialysis patients and 65 individuals with functioning renal transplants. It operates an 18-station dialysis unit, including six cubicles. Nurses play a critical role, delivering acute and chronic dialysis, patient education, anaemia management, and post-transplant care.


According to Jersey's national registry, approximately 2,720 residents (3% of the population) have CKD stages 3–5, with a mean age of 77 years. This prevalence is notably lower than the UK average of 6.1% among adults.

Health and Care Jersey is a directorate of the Government of Jersey that is responsible for the delivery of specialist health care and the running of the general hospital. The directorate is led by a Chief Officer, Tom Walker who leads an executive team including a Director of Clinical Service, Claire Thompson and a Medical Director, Simon West who is also Medical Responsible Officer/Suitable person with the GMC for appraisal and revalidation. Medical Services Care Group

Medicine in Jersey is fully age and specialty integrated. There are 4 medical wards, Plemont, Rozel, Bartlett and Corbiere. A 5-bedded Enhanced Care Area is sited on AAU and there is a separate intensive care unit led by intensivists. There is also an Ambulatory Emergency Care Unit led by the Acute Medicine Department. There is a 25 bed Acute Assessment Unit.

 

SAS doctors are responsible for the day-to-day management of their patients on the wards with the help of clinical fellows, FY1 or FY2 doctor whom they closely supervise.

 

On call, the SAS doctor is responsible for seeing patients referred by the Emergency Department and reviewing patients admitted to the AAU by GPs in conjunction with the Acute Medicine team. A core responsibility of the Medicine Care Group is the provision of a safe acute medical service. The Care Group always provides named Consultant supervision for non-Consultants and ensures that prompt review and treatment is provided for acute medical emergencies referred by General Practitioners or the Emergency Department.

 

Consultants are responsible for producing and updating guidelines for the management of acute medical emergencies in their specialist area.

In addition to outpatient clinics and 4 general medical wards containing 83 beds, the Medical Service Care Group is involved in the management of several specialist facilities. These include:

Proposed Job Plan

All Consultants have a standard 10 PA contract for public work. Within this, all Consultants have 2 core SPA for CPD activities in line with AoMRC guidance. Most Consultants will have a further 1 SPA for agreed activities which support HCJ.


The final job plan will be agreed with the Clinical Lead the Chief of Service for Medical Services, Hospital Director and ratified by the Medical Director. The post holder’s job plan will be reviewed annually and will be appraised according to the current guidance.


An indicative weekly timetable on a 1 in 2 basis for urgent inpatient attending/referrals and urgent laboratory work.


Please note within 3 months of commencement in post, you are expected to agree with your clinical manager a prospective job plan.

 

The final job plan will be agreed with the Clinical Director and Chief of Service, before final approval with the Medical Director. The post holder’s job plan will be reviewed annually and will be appraised according to the current guidance.


In the event of inability to agree a job plan either at appointment or upon annual review, or in the event of issues arising from the appraisal process, negotiation will be with the Chief of Service.

Qualifications and Skills

Essential: 

Primary Medical Degree

Higher Specialty Qualification: MRCP or equivalent

Full Registration with the General Medical Council (GMC) with a license to practice (for overseas candidates it is mandatory that a license is obtained prior to appointment)

Inclusion on the GMC Specialist Register/Certificate if Completion of Training (CCT) or equivalent in renal or be within 6 months of obtaining this the date of the interview – documentary evidence of this must be provided to support your application.

Clinical Experience in similar NHS environment or equivalent

Evidence of adequate training and/or experience as CCDC

Applicants that are non-UK trained, will be required to show evidence of equivalence to the UK CCT
