Job Overview

As an autonomous practitioner, the post holder will lead on clinical care and collaborate with multiple disciplinary teams to provide evidence based, person centred, high-quality diabetes care for the inpatient diabetes service.

 

Aims of the role are to reduce admission to hospital, reduce the length of hospital stay, provide safe discharge from hospital, and consistently strive to improve the safety and experience of people living with diabetes, who access hospital care at Jersey General Hospital.

 

The Clinical Nurse Specialists role will include;

 

Triage and caseload management, specialist clinical assessment of people with diabetes in the inpatient environment, recommending and planning treatment and care, monitoring and evaluating care delivery. Agreeing on safe post discharge follow up, and referral to GP, family nursing, agency or liaising with Diabetes outpatient services to provide ongoing diabetes care.

 

Educating patients on and promoting self-management of diabetes; assessment, provision and use of diabetes self-monitoring equipment (glucose/ketone meters, continuous glucose monitors and basic use of insulin pump therapy), education on diabetes medications, new insulin initiation and medication adjustments.

 

The post holder will provide high quality training to healthcare professionals. Contribute to Diabetes MDT and clinical governance meetings, develop clinical guidance with the aim of striving to improve the safety culture within diabetes care.
