Job Overview

We are looking for a dynamic individual, willing to learn and expand their skills in diabetes care.

 

The aim of the diabetes service is to provide an exceptional standard of specialist care for people with all types of diabetes, at any stage on their diabetes journey. The role of the diabetes specialist nurse is exciting and varied, with a focus on supporting the person with diabetes in self- management.

 

Education is a key part of the role – this could be in the form of structured education (DAFNE/DESMOND/VICTOR) or individual education to support self management. We also deliver group education to hospital staff, community staff and GPs, and relatives/carers.

We provide care and support with newly diagnosed, and ongoing care for existing type 1 diabetes. We initiate and provide ongoing management with various tech and devices – such as insulin pumps, smart multiple-daily-injection systems, and continuous glucose monitoring.

 

We support GP led care in type 2 diabetes by providing structured education soon after diagnosis, and for initiation of injectable therapy if needed. Type 2 patients requiring specialist input on a temporary or longer term basis are on our caseload as are other types of diabetes such as gestational, secondary or genetic.

 

Our team encompasses an inpatient service, aiming to improve the experience of people with diabetes when admitted to hospital.

 

 

Job Purpose

Whilst the post is for the adult service, we work closely with our paediatric colleagues and offer transitional clinics for people moving from the paediatric to the adult caseload and we are frequently liaising with other agencies such as GPs/hospice/midwives/ district nurses.

 

The successful applicant ideally will have 3 years post graduate experience in a medical area. There is an expectation that they learn from patients and colleagues, complete training in diabetes and become competent as an independent practitioner in the above areas. They will be supported in doing this, and competencies will be based on the Trend Diabetes Integrated Career and Competency Framework for Adult Diabetes Nursing. Some training will be off Island.

 

The post is varied, so the person needs excellent organisational skills and the ability to prioritise workload

 

The post is full time, initially based at EQ (Mon-Fri 9-5), but occasional days could be at JGH and occasional days in the community.
