Job Overview
An exciting opportunity has become available for a dynamic and motivated nurse to join our Cardiology team as a Heart Failure Nurse Specialist. You will work closely with doctors, nurses and AHPs to optimise and manage the care of patients with heart failure.
The post holder will provide day to day clinical assessment and care to this patient group and support and advise patients and their carers. The post holder will demonstrate autonomy and professionalism whilst also seeking support from the wider Cardiology team where necessary. Patient’s needs must be placed at the centre of care delivery whilst providing assurance that standards of clinical care is of the highest standard attainable. This is an excellent opportunity for someone who has the motivation and interest to contribute to the management of this patient group.
Applicants should have cardiology experience and be willing to learn the important principles of heart failure management. Support will be provided within the team. Applicants should have some knowledge relating to the management of patients with heart failure.
This role will involve the care, assessment, and management of patients with heart failure in a variety of settings and the successful applicant will need to hold a current and valid driving license. This role will encompass review of patients on the hospital wards, in community settings, in their own homes and in outpatients clinics.
