Job Overview

We are looking for a Learning Disability Liaison Nurse to lead and support high-quality, person-centred care for people with learning disabilities across hospital, primary and acute services on the beautiful island of Jersey.

The role involves working closely with multidisciplinary teams to improve patient experience, ensure reasonable adjustments are in place, and promote seamless care between services. You will provide specialist advice, education and direct support to staff, patients and families, helping to reduce health inequalities and ensure safe, effective care throughout the patient journey. All within access to beautiful beaches, costal walks, and access to numerous outdoor activities in a safe and peaceful environment.
