Job Overview
We are currently seeking a Social Worker to become part of our dedicated Community Mental Health Team Based in St Helier and serving the population of Jersey. You will be allocated care coordinator/lead professional for a caseload of service users with severe and enduring mental health difficulties.
Provide a comprehensive care plan and ongoing assessment to the adults with severe and enduring mental health difficulties in Jersey and their carers, delivering the day to day working of the CMHT and accessing and facilitating resources so as to manage risk and meet identified needs.
The post holder will have to comply with all relevant States Laws, such as the Health and Safety at Work (Jersey) Law 1989 and any other associated legislation, Standing Orders, Financial Directions, and other relevant Codes of Practice, in respect of managing the key project deliverables of programmes and costs through the active management of procurement, approval, financial and administrative procedures. The post holder will also be complying with the Capacity & Self Determination (Jersey) Law 2016 & Mental Health (Jersey) Law 2016.
