Job Overview

We are seeking a compassionate and motivated Social Worker to join our Learning Disability Services team, supporting adults to lead safe, independent, and meaningful lives. This role focuses on delivering person-centred, strengths-based social work practice, ensuring individuals with learning disabilities are empowered to make informed choices and achieve positive outcomes.


You will undertake comprehensive assessments, develop and review care and support plans, and manage a varied caseload, including individuals with complex needs. Working closely with families, carers, and multi-disciplinary partners, you will coordinate effective support, promote inclusion, and advocate for individuals’ rights.


The role includes safeguarding responsibilities, requiring you to identify, assess, and respond to risks while ensuring the protection of vulnerable adults in line with legislation and best practice. You will maintain accurate records, contribute to service development, and support a culture of continuous improvement.

Job Purpose

The role involves working in partnership with families, carers, and multi-disciplinary professionals to coordinate effective, joined-up support that meets complex needs.

You will take a key role in safeguarding, identifying and managing risks while upholding individuals’ rights and ensuring compliance with relevant legislation and professional standards. 

Maintaining accurate records and contributing to service development and continuous improvement are essential.

This role requires confident decision-making, professional accountability, and the ability to balance autonomous practice with effective teamwork to achieve positive outcomes.

Statutory Responsibilities

Assessment of Need: Undertake holistic assessments in line with Jersey’s adult social care framework, identifying needs, strengths, risks, and required outcomes.
 
Care Planning and Review: Develop, implement, and regularly review person-centred care and support plans, ensuring they promote independence, wellbeing, and community inclusion.
 
Safeguarding Adults: Identify and respond to risks of abuse or neglect in line with Jersey Safeguarding Adults procedures, contributing to enquiries and protection planning where required.
 
Capacity and Consent: Apply the Capacity and Self-Determination (Jersey) Law 2016, assessing capacity, supporting individuals to make their own decisions, and facilitating best interests processes when necessary.
 
Care Coordination: Act as care coordinator within the Community Learning Disability Team (CLDT) where required, ensuring effective multi-agency collaboration and continuity of care.
 
Risk Assessment and Management: Balance risk and autonomy, promoting positive risk-taking while ensuring appropriate safeguards are in place.
 
Advocacy and Rights: Promote human rights, equality, and access to independent advocacy, ensuring individuals’ voices are central to all decisions.
 
Professional Accountability: Maintain clear, accurate, and timely records in line with professional standards and organisational governance requirements.
 
Policy and Legal Compliance: Ensure practice aligns with Government of Jersey policies, safeguarding frameworks, and professional regulatory standards (e.g. SWE/HCPC principles as applicable).

Qualifications

Professional Social Work qualification (Degree/Diploma in Social Work or equivalent).
 
Current registration with a recognised regulatory body (e.g. Social Work England or equivalent)
 
Evidence of continuing professional development

Knowledge

Experience of working with adults with learning disabilities, including those with complex needs.
 
Knowledge of safeguarding adults procedures and risk management.
 
Understanding of person-centred and strengths-based approaches to social work practice.
 
Experience of assessment, care planning, and care coordination within a multi-disciplinary setting.
 
Knowledge of the Capacity and Self-Determination (Jersey) Law 2016 and its application in practice.
 
Experience of multi-agency working and partnership collaboration

Technical Work Based Skills

Skilled in undertaking comprehensive assessments and formulating person-centred, strengths-based care and support plans.
 
Ability to apply the Capacity and Self-Determination (Jersey) Law 2016, including capacity assessments and best interests decision-making.
 
Experience in safeguarding practice, including risk identification, enquiries, and protection planning.
 
Effective care coordination within a multi-disciplinary (CLDT) environment, ensuring integrated and cohesive support.
 
Strong case management and recording skills, producing clear, accurate, and defensible documentation aligned with professional standards

Experience

  • Experience of working with adults in a health or social care setting, with an understanding of the needs of people with learning disabilities

  • Awareness of safeguarding adults principles and risk management in practice
  • Experience of working collaboratively with families, carers, and multi-disciplinary teams
  • Understanding and application (or willingness to develop) knowledge of the Capacity and Self-Determination (Jersey) Law 2016
