Job Purpose

The full-time post of Staff Grade in Obstetrics & Gynaecology comprises 10-programmed activities per week, plus 4 additional programmed activities based on the resident out of hours rota at the General Hospital, Jersey, assisting in providing a 24:7 Obstetrics & Gynaecology service for the population of Jersey.

The appointee will be expected to participate in the Middle Grade resident out of hour’s rota 1:8 on-call and day-time sessions will be flexible so that they reflect service needs, at the discretion of the Head of Service and Clinical Lead. The post holder will be on duty with a Resident Doctor.

The post holder should be experienced in all aspects of Labour ward management and operative deliveries.

General gynaecology experience at Staff Grade or Registrar level (or equivalent) is essential.

There is an Assisted Reproduction Unit with a Satellite Assisted Conception service linking in with the Lister Hospital in London.  Clinics are held to prepare couples for the Assisted Conception programme in accordance with the HFEA. 

There are 2 Colposcopy clinics each week and it would be desirable if the successful candidate was accredited in Colposcopy. 

An early pregnancy unit is held Mon – Fri mornings and experience in early pregnancy scanning is desirable. There are plans to develop this into an EPAU.

Statutory Responsibilities

The unique location, geography and constitution of the Island means that the Department provides a much wider range of services than would be expected in a comparative health authority or Social Services in the United Kingdom. 

The post holder will be assigned general clinical duties to include attendance at out-patient clinics and theatre sessions.  Specialist skills such as Infertility, Early Pregnancy Scanning, or Colposcopy would be advantageous, and these skills would be included in the weekly timetable.

The post holder will provide the appropriate level of support to the Consultant Obstetricians and Gynaecologists in the care of the patients.

Jersey is fortunate to have maintained a team structure so the post holder would be assigned to work with a named consultant.

The post holder will undertake the administrative duties associated with the care of their patients and the efficient running of the department, ensuring that up-to-date, accurate, legible and comprehensive signed notes are recorded in the patient’s case notes.

The post holder will prescribe appropriate medication and therapy, ensure that any procedure for patient management is undertaken at the most appropriate time and in the most appropriate location, promoting patient dignity and privacy. The post holder will be required to undertake and pass Health and Care Jersey e-Prescribing course.

 

The post holder will support the Consultant Obstetricians and Gynaecologists in the development, review and updating of protocols and practice regarding the management of obstetric and gynaecological patients.

Close links are maintained with the Wessex Deanery and other major centres in the UK and consultants in areas such as oncology, neurology and cardiology visit the Island on a regular basis.  The post holder will be expected to participate in the training of our Resident doctors and Medical students for whom they are responsible and ensure that training and learning needs are identified and facilitated.

The post holder will be expected to take part in management and audit activities and represent the service at Hospital meetings, as agreed with colleagues. The post holder will be expected to be fully involved in a ‘Risk Management” programme and support the Governance agenda.

The post holder will deputise from time to time for absent colleagues, performing additional duties in occasional emergencies and unforeseen circumstances; being available for such irregular duties is essential for the continuity of patient care.

Doctors are expected to be flexible and to cooperate with reasonable requests to cover for their colleagues’ absences where it is practicable to do so. Where doctors undertake such duties in addition to their contracted hours they will receive appropriate remuneration.

The post-holder will be indemnified by the States of Jersey for all public work undertaken as part of their Contract of Employment.  The provision of indemnity cover for any other medical activities will be the responsibility of the post-holder.

The successful candidate should be adaptable, flexible and innovative in order to meet the ongoing challenges within the service. The responsibilities of the role may change from time to time.

The postholder will need to continue to meet all regulatory arrangements relating to registration, licensing and revalidation.

The post holder will be required to participate in an on–call rota with prospective cover, comprising of 7 middle grade doctors currently and once this post is recruited to will increase to (1:8) and provide a structured handover to colleagues to ensure continuity of care for patients. There is also a resident doctor rota (full shift pattern with 6 doctors) and a consultant on call rota (1:6) in place.

Essential Qualification and Registration requirements

Full GMC registration with license to practice (for overseas candidates it is mandatory that a licence is obtained prior to appointment).

Primary Medical Degree
Part 1 MRCOG (knowledge of human Anatomy, Physiology, pharmacology and Biochemistry and their application to the clinical practice of Obstetrics and Gynaecology.)
Current ALS certificate

Essential Experience

4 years post-graduate Medical Training

3 years experience in an approved Obstetrics and Gynaecology Training environment, 2 of which must be at Registrar/Middle Grade level or equivalent

Evidence of Competency to the level set out for training in MRCOG

Relevant clinical experience in an NHS environment

Basic legal requirements for consent and note taking

Previous exposure to a busy labour ward and ability to perform operative deliveries

Gynaecological practice and current treatments to be encountered for patients presenting to the department in the specialty

Job plan

This is a full-time post of 40 hours (10 PAs) per week, plus additional APA's. Work commitments and intensity will be reviewed at least annually through the job planning process. Any substantial changes in the nature of the post will be agreed through the job planning process.  The post holder’s job plan will be reviewed annually and will be assessed according to current guidance.  The postholder will be managerially responsible to their supervising consultant and through them to the Lead Consultant for the service.

The SPA will include: CPD, audit & quality improvement activity, appraisal, revalidation and PDP groups.  The balance of PAs and SPAs may vary by agreement between the organisation and the post holder but will not be less than one SPA. 

The post holder will be provided with a shared office space including telephone and desktop. 

Shortly after appointment a timetable will be agreed between the Lead Consultant and the post holder.
