The Post

We are pleased to be able to offer specialty posts in our medical teams for Respiratory Medicine, Care of the Elderly and Renal Medicine, committing to the assessment and ongoing care of medical patients in Jersey General Hospital. The post holder will be a member of the medical team and will be able to perform specialist duties in areas such as Acute Medicine, Renal Medicine, Respiratory Medicine working directly with those consultant teams. The post holder will be pivotal in the clinical management of presentations to Jersey General Hospital.

 

The post holder will work across all domains of Medicine including:

-Medical Wards 

-Specialist Teams and Outpatients 

-Ambulatory SDAC areas where appropriate 

Respiratory Specialty

Duties involve care of inpatients on a general/respiratory ward and those on High Dependency Unit and Intensive Care Unit. There will be the opportunity for some outpatient activity. The successful candidate will be expected to have completed core medical training or its equivalent and have achieved MRCP diploma or its equivalent. Specialist knowledge in Respiratory Medicine is expected, although there is a team of respiratory consultants to support.

Renal Specialty

The post holder will be responsible for delivering clinical care on the Renal Unit and in outpatient clinics. During periods when the post holder is on take, routine outpatient duties will be suspended. At these times, the post holder is expected to take a lead role in coordinating the assessment and management of patients referred by General Practitioners, the Emergency Department, and other hospital services. This includes both the initial assessment of some patients and the senior review of those previously seen by junior members of the team. The role includes participation in specialist and procedural clinics within Renal Medicine, along with clinical oversight and support for patients receiving care in the renal dialysis unit.

Care of the Elderly Specialty

This Staff Grade position offers a rewarding opportunity to support the delivery of high-quality care to older adults within both inpatient and outpatient settings. The post holder will work as part of a multidisciplinary team managing patients with complex comorbidities, frailty, and age-related conditions. Key responsibilities include participating in ward rounds, undertaking comprehensive geriatric assessments, contributing to discharge planning, and supporting rehabilitation and long-term care strategies.

Duties

- To undertake clinical practice within the medical wards ensuring that personalised, evidence informed medical care plans are delivered to all patients and that they are appropriately documented and regularly evaluated.

- To work in close partnership and liaison with Primary Care to provide telephone advice including provision of hospital avoidance initiatives.

- To undertake inpatient management of medical patients including participation on ward rounds and board rounds.

- To provide clinical advice & support to primary care colleagues and other consultants within Health & Care Jersey medical model.

- To assist, where appropriate, with the management of patients of a higher acuity within the Enhanced Care Area.

- To provide advice and timely medical reviews and interventions as part of the interdisciplinary model of care.

- To engage with wider members of the MDT.

- To liaise with tertiary referral centres as appropriate to provide patient centred care & service improvements as directed by consultant plans.

- Maintain good quality up to date medical records including timely discharge letters and summaries.

- To ensure appropriate documentation and coding is used to provide consistent audit and monitoring of the service.

- To adhere to local prescribing guidelines.

- The post holder should be adaptable, flexible and innovative in order to meet the ongoing challenges within the service. The responsibilities of the role may change from time to time.

- To comply with infection prevention & control requirements including the latest COVID-19 guidance and policies.

- The post holder will be expected to be fully involved in a ‘Risk Management” program and support the Governance agenda.

Teaching and Research

The department is recognised for junior doctors from the Wessex deanery and medical students from Southampton and active teaching is encouraged in this role. It is expected that the post holder is participating in the training of Medical staff and that the candidate will ensure that the training and learning needs are identified and facilitated. This includes educational support for the clinical fellows recruited directly to Health & Care Jersey department.

 

The post holder should deliver training to a range of professionals across the multi-disciplinary team in stroke medicine to create a service of excellence for Jersey.

 

Opportunities for participating in research are supported when appropriate. The Health & Care Jersey department has a policy for professional leave and is supportive of external duties for the good of the wider profession.

On-Call Rota

The on-call commitment is 1:8, this includes weekends and night cover.

Medical Services Care Group

Medicine in Jersey is fully age and specialty integrated. There are 3 medical wards, Plemont, Rozel and Corbiere. A 5-bedded Enhanced Care Area is sited on AAU and there is a separate intensive care unit led by intensivists. There is also an Ambulatory Emergency Care Unit led by the Acute Medicine Department. There is a 25 bed Acute Assessment Unit.

 

SAS doctors are responsible for the day-to-day management of their patients on the wards with the help of clinical fellows, FY1 or FY2 doctor whom they closely supervise.

 

On call, the SAS doctor is responsible for seeing patients referred by the Emergency Department and reviewing patients admitted to the AAU by GPs in conjunction with the Acute Medicine team. A core responsibility of the Medicine Care Group is the provision of a safe acute medical service. The Care Group always provides named Consultant supervision for non-Consultants and ensures that prompt review and treatment is provided for acute medical emergencies referred by General Practitioners or the Emergency Department.

 

Consultants are responsible for producing and updating guidelines for the management of acute medical emergencies in their specialist area.

 

Care Group Statistics

 

There are 63 medical beds. 12-20 acute medical admissions on average during each 24-hour period on call within a range of approximately 6-24. There are approximately 4,500 medical admissions annually. In addition to outpatient clinics and 3 general medical wards containing 63 beds, the Medical Service Care Group is involved in the management of several specialist facilities. These include:

 

25 bedded Acute Assessment Unit (AAU)

5 bedded Enhanced Care Area (ECA)

A well-equipped Clinical Investigations Department providing echocardiography, exercise ECG, respiratory assessment and GI physiology.

An endoscopy suite providing a full range of endoscopy and bronchoscopy services.

Diabetes Centre providing outpatient care for the diabetic population of Jersey, including retinal photography and specialist nursing support.

Renal/Dialysis unit, providing emergency, acute and longer-term renal support as well as management of transplanted patients and assessment and treatment of renal conditions.

6 bedded isolation area for infected or neutropenic patients.

Primary Medical Degree

Full Registration with the General Medical Council (GMC) with a Licence to practise (for overseas candidates it is mandatory that a licence is obtained prior to appointment).
