The Post

The post holder will work as part of the clinical (medical and nursing) team, and under the supervision of the Consultant Haematologists, to provide a comprehensive clinical haematology service.

Duties

The duties outlined below are not definitive and may be changed in accordance with the needs of the service. This includes the following, which is not an exhaustive list; the activities and responsibilities will not only reflect the experience of the candidate, but also their interests and future career intentions.

 

To assist Consultant staff in the management of adult Haematology patients, both malignant and non-malignant, in the outpatient, day case and inpatient setting.

 

• Attend and assist at Outpatient clinics.

• In conjunction with other members of the team, to manage haematology inpatients and to conduct ward rounds.

• To undertake bone marrow and other procedures as necessary and assist with reporting of bone marrows and blood films.

• Provide medical support for chemotherapy and day unit services.

• To be available for advice to junior doctors, attending ward/day unit as necessary.

• To liaise with GPs and provide advice in consultation with Consultant Haematologist.

• To assist with, attend and contribute to patient management at MDTs.

• To assist with laboratory queries and report films, bone marrows within your level of competence and training. Training would be encouraged.

• To ensure all documentation is comprehensive, accurate and completed in a timely manner.

• Promote a friendly and team based working environment – in line with our hospital values.

• Teaching of primary care colleagues and junior medical staff through mixture of case-based review and formal teaching activity.

Haematology Department

Jersey General Hospital (JGH) currently offers a wide range of clinical and laboratory haematology services. Adult Haematogical Oncology is delivered to BCSH Level 2a as defined by the BCSH Guideline (2010). Patients requiring BCSH Level 2b care and above including the management of acute leukaemia, autologous stem cell transplantation and plasma exchange therapy are transferred electively or as emergencies to Royal Bournemouth Hospital.

 

There are strong links with University Hospitals Southampton (UHS) for clinically aggressive lymphoma, allogeneic stem cell transplant and radiotherapy. A lymphoma consultant from UHS is contributing to the lymphoma service on a fortnightly basis. UHS also hosts the regional Teenage and Young Adult (TYA) Haematology-Oncology service. There are also links with BRH for Leukaemia induction and King’s Partners, London for specialist tertiary referrals.

 

The Haematology Department is undergoing significant restructuring, and the successful appointee will be expected to contribute to shaping developments. There are opportunities for further service expansion with the potential to develop academic activities and strengthen external links.

Multi-Disciplinary Blood Cancer Team Meetings (MDTMs)

A Jersey-Guernsey local Haem-Oncology MDTM is held on site every Tuesday morning. The UHS Lymphoma Network MDTM occurs in Southampton every Tuesday morning. The Dorset Cancer Network Blood Cancers MDTM operates each Friday morning including our Bournemouth referral partners. All MDTs are attended via video link.

 

Clinical Facilities – outpatient & day-care

A day care Chemotherapy Unit on Le Quesne Unit is shared with Medical Oncology. Le Quesne Unit also hosts haematological oncology outpatient clinics including lymphoma clinic. A separate general Medical Day Care Unit hosts an infusion/transfusion service. General Haematology with some haem-Onc Clinics is currently held in the Medical Outpatient area.

 

Clinical Facilities – Inpatients

Inpatients with blood disorders are managed on general medical wards and supervised hour-to-hour by junior medical staff working in teams led by Physicians-with-an-Interest. Shared care local Specialist Haematology input runs in office hours. Out-of-hours Consultant Haematologist cover is by telephone support from colleagues at Royal Bournemouth Hospital (RBH) well established and any patient admitted who cannot reasonably be managed at night/weekends by a general medical junior doctor team are transferred to specialist Haematology beds on ward 11 at RBH.

Haematology Department

Haematology &Transfusion Laboratory Service

The Haematology & Transfusion Laboratory function is currently embedded in a Blood Sciences Department and provides a routine service to our users from 0800 hours to 1800 hours with a structured out of hours service at other times, provided by a multidisciplinary out-of-hours team. The service provides a maximum 12-hour turn-around time for all routine work undertaken on-Island and a one hour turn around for all emergency work. UK reference centres are used for analyses that are not provided currently in Jersey including Flow-cytometry, Cytogenetics, Molecular Genetic studies and the assessment of abnormal haemoglobins. The Laboratory Service hosts and supports and independent Blood Donor Service that routinely allows Jersey to be 95-96% selfsufficient in red cell provision. The Island also benefits from an unusually high repeat donation rate. There is no formal service agreement with a Specialist Integrated Haematological Diagnostic Services (SIHMDS) centre for integrated reporting. The bone marrow samples are split and sent to 3 different centres for diagnostic input. There is also no related histopathological MDT input for local MDT.

 

Therapeutic Venesection Service

There are a substantial number of Haemochromatosis patients in Jersey because of active case finding, together with a significant number of Myeloproliferative Neoplasm (MPN) patients requiring therapeutic venesection. Of the overall 1.66 FTE Donor Nurse, 0.4FTE is currently committed to therapeutic venesection of patients with Haemochromatosis and Myeloproliferative Neoplasms.

 

Nurse-led Anticoagulant Dosing Service

The advent of the new generation of Directly Acting Oral Anticoagulants (DOACs) has resulted in a significant decline in Warfarin use and need for INR dose monitoring. There are many complex patients managed by a part time anticoagulant nurse.

Medical Services Care Group

Medicine in Jersey is fully age and specialty integrated. There are 3 medical wards, Plemont, Rozel and Corbiere. A 5-bedded Enhanced Care Area is sited on AAU and there is a separate intensive care unit led by intensivists. There is also an Ambulatory Emergency Care Unit led by the Acute Medicine Department. There is a 25 bed Acute Assessment Unit.

 

SAS doctors are responsible for the day-to-day management of their patients on the wards with the help of clinical fellows, FY1 or FY2 doctor whom they closely supervise.

 

On call, the SAS doctor is responsible for seeing patients referred by the Emergency Department and reviewing patients admitted to the AAU by GPs in conjunction with the Acute Medicine team. A core responsibility of the Medicine Care Group is the provision of a safe acute medical service. The Care Group always provides named Consultant supervision for non-Consultants and ensures that prompt review and treatment is provided for acute medical emergencies referred by General Practitioners or the Emergency Department.

 

Consultants are responsible for producing and updating guidelines for the management of acute medical emergencies in their specialist area.

 

Care Group Statistics

 

There are 63 medical beds. 12-20 acute medical admissions on average during each 24-hour period on call within a range of approximately 6-24. There are approximately 4,500 medical admissions annually. In addition to outpatient clinics and 3 general medical wards containing 63 beds, the Medical Service Care Group is involved in the management of several specialist facilities. These include:

5 bedded Enhanced Care Area (ECA)

A well-equipped Clinical Investigations Department providing echocardiography, exercise ECG, respiratory assessment and GI physiology.

An endoscopy suite providing a full range of endoscopy and bronchoscopy services.

Diabetes Centre providing outpatient care for the diabetic population of Jersey, including retinal photography and specialist nursing support.

Renal/Dialysis unit, providing emergency, acute and longer-term renal support as well as management of transplanted patients and assessment and treatment of renal conditions.

6 bedded isolation area for infected or neutropenic patients.

Qualifications and Skills

Primary Medical Degree

Full Registration with the General Medical Council (GMC) with a Licence to practise (for overseas candidates it is mandatory that a licence is obtained prior to appointment).
