Job Overview

We are seeking a dedicated and motivated Support Worker to join our small and supportive Transition Team within Adult Mental Health in Jersey. This role works closely with young people aged 18–25 who are transitioning from CAMHS into adult services, supporting them to build independence and move forward in their mental health recovery. The ideal candidate will be a hard‑working, compassionate individual who is passionate about empowering young adults during this important stage of their lives. You will be part of a close‑knit team committed to person‑centred care and positive outcomes. Applicants must be entitled to work in Jersey to apply.

Job Purpose

Your day to day roles and duties will include the following 

  • Mental Health Psychoeducation 
  • Teaching and demonstrating coping skills and strategies 
  • Practical support towards independent life skills
  • Supporting community engagement
  • Supporting young people into education, employment or training 

You will need to be a person who can work closely with the Multidiscplinery team to manage positive risk taking in the community, meeting each individual where they are at in their recovery journey. You will have the opportunity to build and develop long term meaningful relationships with the young people you support. You will be well supported by a hard working, passionate and enthusiastic team to ensure the best outcomes for the young adults.
