Job Overview

An exciting opportunity has arisen within the Emergency Department at Jersey General Hospital. We are seeking to appoint a dynamic and forward thinking staff grade doctor to join our dedicated Emergency Medicine team. This position compromises 14 PAs and does not include on-call commitment.

The Post

The Emergency Department team consists of 9 Consultants supported by 15 Middle Grade doctors (including these posts), 5 Foundation Yr. 2 and 3 GP trainees and 7 clinical fellows (a total of 15 SHO junior rota) & 5 ENPs (ENP provide 9am-9pm 7 days a week service). With 40,000 attendances per year and no other hospital on Island the Emergency Department provides exposure to the full range of acute emergency presentations. The appointee will contribute to the Middle Grade tier of the Emergency Department Rota which includes 24/7 cover. The post also attracts 1 Support Professional Activity and Study leave allocation. The post holder will be a tier 4 staff grade therefore will at times have to be in charge of the department without a consultant being on-site.

Duties

The post holder will ensure the provision of high-quality emergency care within the Emergency Department. The post holder will be expected to be competent in the management of the normal range of emergency medical conditions and injuries and will be expected to supervise junior trainees and medical students. Staff grade doctors are also asked to assist at follow up clinics within the ED. The post holder will be expected to take operational responsibility for the department as required and in particular, when they are the most senior doctor present, such as weekends and evenings and overnight providing a role model to the junior medical staff and nurses. On designated days the post holder will be responsible for checking and acting on X-ray and pathology results, and for validating discharge letters.

Rota

The middle grade tier operates a self-rostering system which is run prospectively for Annual Leave and Study Leave. This allows a greater degree of flexibility than a rolling rota. This post will cover early, late & night cover seven days a week

Clinical Governance and Departmental Development

Along with the Consultants the post holder will be expected to participate in the delivery of good clinical governance. Specifically, the post holder will liaise with senior doctors and members of other departments in the development of multidisciplinary, evidence-based guidelines, protocols and care pathways. The post attracts 1PA for Supporting Professional Activity and a Study leave allocation. The post holder will perform duties and CPD requirements to fulfil appraisal requirements and to contribute to the non-clinical activities of the department.

- The post holder will have an ED Consultant nominated as an Educational/ Clinical Supervisor to support these activities.

- The post holder will be expected to participate in the training of Medical Staff for whom he/she is responsible for.

- The post holder will ensure that training and development needs are identified for junior doctors who they supervise.

- Junior doctors will be appraised at appropriate intervals.

- The post holder will be expected to take part in management, quality improvement and patient safety activities whilst representing the department at Hospital meetings, as agreed with colleagues.

- The post holder will be expected to be fully involved in a ‘Risk Management” programme and support the Governance agenda.

- The successful candidate should be adaptable, flexible and innovative in order to meet the ongoing challenges within the service.

On Call Rota

There Is no on-call rota.

Emergency Medicine

The Emergency Department is self-contained and well situated close to support services. It is equipped to a high standard and has had a recent refurbishment which provides a delightful bright environment to work in opposite one of St Helier’s parklands. Jersey is currently receiving significant inward investment in its health service which is completely independent of the NHS. Being an island-based emergency department, there is exposure to all types of emergency work. There are also occasions when patients need to be air-evacuated to a main land hospital. For this we have access to three aircraft fully dedicated to aeromedical evacuation. The JETS (Jersey Emergency Transfer Service) team will provide training for doctors involved in island to mainland transfers. Doctors involved in aero-medical transfers are paid an additional fee per transfer.

Medical Services Care Group

Medicine in Jersey is fully age and specialty integrated. There are 3 medical wards, Plemont, Rozel and Corbiere. A 5-bedded Enhanced Care Area is sited on AAU and there is a separate intensive care unit led by intensivists. There is also an Ambulatory Emergency Care Unit led by the Acute Medicine Department. There is a 25 bed Acute Assessment Unit.

SAS doctors are responsible for the day-to-day management of their patients on the wards with the help of clinical fellows, FY1 or FY2 doctor whom they closely supervise.

On call, the SAS doctor is responsible for seeing patients referred by the Emergency Department and reviewing patients admitted to the AAU by GPs in conjunction with the Acute Medicine team. A core responsibility of the Medicine Care Group is the provision of a safe acute medical service. The Care Group always provides named Consultant supervision for non-Consultants and ensures that prompt review and treatment is provided for acute medical emergencies referred by General Practitioners or the Emergency Department.

Consultants are responsible for producing and updating guidelines for the management of acute medical emergencies in their specialist area.

There are 63 medical beds. 12-20 acute medical admissions on average during each 24-hour period on call within a range of approximately 6-24. There are approximately 4,500 medical admissions annually.

A well-equipped Clinical Investigations Department providing echocardiography, exercise ECG, respiratory assessment and GI physiology.

An endoscopy suite providing a full range of endoscopy and bronchoscopy services.

Diabetes Centre providing outpatient care for the diabetic population of Jersey, including retinal photography and specialist nursing support.

Renal/Dialysis unit, providing emergency, acute and longer-term renal support as well as management of transplanted patients and assessment and treatment of renal conditions.

6 bedded isolation area for infected or neutropenic patients.

Infection Control

Qualification and Skills

Essential: 

Primary Medical Degree

Full Registration with the General Medical Council (GMC) with a license to practice (for overseas candidates It Is mandatory that a license Is obtained prior to appointment)

 

Desirable:

Clinical experience in a similar environment

General Experience

Essential: 

Evidence of Emergency Medicine experience in an appropriate setting at Staff Grade / Senior level at least 12months full time (or equivalent part time)

Completed at least four years’ full-time postgraduate training (or its equivalent gained on a part-time or flexible basis) at least two of which will be in a specialty training programme in a relevant specialty or as a fixed term specialty trainee in a relevant specialty.

Specialist Experience

Essential: 

Provider status in ATLS/ETC (or equivalent)

Provider status in ALS (or equivalent)

Provider status in EPLS/APLS (or equivalent)

MRCEM or FRCEM (or equivalent)

MRCP

 

Desirable:
