We are looking to recruit a team of 4 Specialist Paramedics as part of our strategy to enhance the care we provide to patients whilst reducing the demand on Frontline colleagues to ensure our patients receive the Right Care at the RIGHT time and at the RIGHT place.

Primarily, workstreams will revolve around a Four R concept- RESPOND-REDUCE-REFER-ROTATE, in which this new team will respond alongside frontlines colleagues as a clinical leader, manage low acuity high complex incidents through ‘see treat and refer’ protocols, become actively involved in reducing the number of ambulances required at incidents through ‘Hear and treat’ workstreams and build stronger referral pathways that better suit the needs of our patients.

Job Purpose

To deliver appropriate high quality Urgent & Emergency Care to all patients on behalf of the States of Jersey Ambulance Service (SoJAS). This will require the managing of treatment of patients who contact 999; whether through face-to-face assessment, re-triage or through a hear, treat and refer model.

To demonstrate and exercise high levels of decision making, discretion and judgement as a Specialist Paramedic in clinical practice to apply professional acumen to make safe, clinical decisions when managing a wide range of patient demographics with undifferentiated conditions or injuries.

Provide clinical triage to the Ambulance Service by referring patients as appropriate to alternative health and social care agencies in cases where calls do not require a hospital attendance.

This post is open to existing qualified Specialist Paramedics or Trainee SP who will work in conjunction with the 4 pillars of advanced practice framework to deliver key fundamental characteristics including: erudite and focused history-taking, enhanced clinical examination, timely investigations, point-of-care testing, and subsequent interpretation of data to inform treatment and referral decisions.

To work as an autonomous practitioner as part of a multi professional team in the delivery of the care as directed by the ambulance service. To attend/contact patients presenting with undifferentiated emergency, urgent and unscheduled care problems, managing patients at the Right Time and the Right Place to enhance the patients experience through their emergency, urgent and unscheduled care journey through the provision of emergency/urgent assessment, diagnosis and treatment. 

To exhibit an advanced breadth and depth of knowledge combined with critical thinking and expertise in patient assessment, point of care patient diagnostics and clinical interventions which will be supported by an extended scope of practice beyond that required for paramedic. 

To develop knowledge and skills in identified areas of specialist practice and contribute to pan-service education, training, research, and audit activity.  To work to a defined medical model with the attitude, skills and knowledge base to deliver holistic care and treatment within a variety of settings with a broadly defined level of autonomy.  

Provide visible leadership, support and management to more junior clinical staff including supervision and performance management. Provide leadership and support (remote & on scene) to other clinicians on duty ensuring patient needs are prioritised appropriately, and clinical support is available when needed.  

Provide a ‘hear and treat service’ using a recognised medical model for patients presenting with undifferentiated, undiagnosed problems. This will mandate use of advanced skills in history taking, physical examination, problem solving and clinical decision making to establish a diagnosis and management plan via remote contact of patients. Actively participate in clinical audit to identify, report and act on any clinical risks, contribute to and apply policies, procedures and guidelines and identify educational needs of self and others and proactively meet these needs through close working with the SoJAS training department.    

Work alongside senior management colleagues to develop and implement innovative ways of working to benefit the whole heath community. Support Senior Managers in reviewing clinical investigations, fitness to practice cases and clinical enquiries. To be the named lead for key strategic SoJAS workstreams as directed by the Manager (i.e. Falls and End of Life). Advise the management team on latest evidence and changes to current practice based on research.     

Assist in the development of an integrated approach to the delivery of emergency care through supporting the development, structure and implementation of interprofessional and partnership working through active liaison with applicable branches of the Government of Jersey.  This will incorporate care pathway bundles, direct referral pathways and holistic care planning for patients, to ensure the patient receives the right care at the right time in the right place.     

Be prepared to respond to immediate life-threatening calls and undertake operational duties as required by the States of Jersey Ambulance service in periods of high demand. Respond to Major Incidents as part of SoJAS requirement and response plan. Maintain an SP Clinical Practice portfolio as well as meeting the CPD requirements of the HCPC and statutory and Mandatory training as directed by the SoJAS Training department including Safeguarding, Conflict Resolution and Moving & Handling. As a new and developing role, SoJAS may alter, where reasonable, the expectations of the role to meet operational requirements or unexpected developments.

Person specification

  • The ability to enhance and attain extensive knowledge of key areas of paramedicine though the attainment of postgraduate certifications in key areas such as Advanced Practice.
  • Demonstrate ability in working in highly complex situations that require rapid decisions and effective decision-making. Being comfortable and confident to analyse situations, create, plan and act independently.
  • Have a compassionate, patient-centred approach that helps build trust and improve patient outcomes and demonstrate effective communication skills to explain complex information and decisions to staff, patients and family,
  • The ability to remain calm in challenging scenarios and maintain composure and focus whilst providing reassurance to patients and colleagues.
  • Be comfortable working as part of a multi-disciplinary team and collaborate confidently with a wide range of health stakeholders.
  • Demonstrate Leadership skills particularly in guiding less experienced colleagues, coordinating care, and advocating for patients effectively.
  • Show commitment to adaptability and lifelong learning and research, demonstrated by a CPD portfolio that evidences up to date practice and a willingness to take on new challenges and adapt to different care settings, coupled with a desire to ensure that this knowledge is imparted to ambulance staff through bespoke and planned training sessions.

Qualifications

Essential:

Registered Paramedic with current HCPC registration.  

Degree in Paramedic Practice or equivalent experience.  

Qualified in ambulance emergency response driving or equivalent.

Desirable:

PGDip Advanced Clinical practice  

Non-Medical Prescriber qualification  

Teaching or Mentorship qualification  

Physical Assessment & History taking module.  

Knowledge

Understanding of the management of long-term conditions & complex medical needs.  

Knowledge of  Infection, Prevention & Control, Safeguarding processes and medicines management  

Knowledge of implementation of Clinical Governance strategy and the process of clinical review.

Good understanding of Datix and health risk reporting.  

 Knowledge of Ambulance Service Protocol, Policy and Procedure.

 

Technical Work Based Skills

IT skills including the use of Microsoft packages, Excel, presentational packages.

General skills / attributes

Good presentation and communication skills to communicate complex, sensitive or contentious information.

Proven ability to communicate effectively both orally and in writing, including producing reports that may be used in legal proceedings.

Ability to work independently using own initiative.

Experience

Minimum of 3 years post qualification experience within an Ambulance Service or equivalent with evidence of continuing professional development

Desirable:

Experience of Preparation and delivery reports and the monitoring of performance and action plans. 

Experience in Remote Triage or Clinical Validation.

Experience in Primary and/or Urgent Care Settings.
