Job Purpose

  • To promote and develop the role of mental health liaison by providing clinical and professional leadership in development and implementation of evidence-based practices to support people with a mental health need who are under the care of the General Hospital.

 

  • To facilitate high-quality person-centred specialist health care for people with mental health needs within a hospital setting.

 

  • To ensure seamless care provision between mainstream hospital wards and mental health services including inpatient and community services.

 

  • Providing nursing advice, education and direct support on complex health care needs of people with a mental health need, families, staff and provider services.

Job Specific Outcomes

  • Responsible for liaising with Hospital wards,  Adult Social Care & Mental Health Service staff and all other relevant carers in order to facilitate an effective health care pathway for people with mental health needs who have been admitted to the JGH.  
  • Enhance care delivery by promoting a whole system approach to achieve positive outcomes within a multi-agency/disciplinary partnership. 
  • Participate in multidisciplinary/agency team meetings, hospital admissions, case conferences, pre discharge meetings, as well as referrals onwards to other mental health teams. 
  • Engage with all health care services to promote the rights of individuals, to access an equitable service and reasonable adjustments are made in order to meet individual needs.  
  • Liaise with Hospital, Acute Services and GP practices to ensure appropriate needs identified and recorded, to carry out routine mental health assessment, screening and consultation for adults with a mental health need where required. 
  • ​​Assess and advise on issues surrounding complex health care needs of people with mental health in relation to emergency or elective admission, access to investigations and diagnostics required for health care needs and for discharge planning.  
  • Provide knowledge and advice to care staff on complex health needs and presentations   (e.g. Dementia, psychotic disorders).   
  • Maintain as part of continual professional development up to date clinical mental health evidenced based knowledge/skills, using information to affect change in practice and ensure dissemination of new knowledge working within equality and diversity guidance, to reduce discrimination and disadvantage by making access to health services equitable.

Qualifications

  • Level 1 Registered Mental Health Nurse (RMN) with NMC registration.  

 

  • Mandatory registration with Jersey Care Commission on appointment and must maintain registration yearly.   

 

  • Professional Degree.

Knowledge

  • Professional knowledge across a range of mental health health related procedures underpinned by theory.  

 

  • Knowledge of relevant local / national policies including, Safeguarding , Mental Health Law, (Jersey) 2016 and Capacity and Self Determination  Law (Jersey) 2016. 

 

  • Skilled practitioner in assessing health needs of individuals.

 

  • Specialist knowledge of the pharmacological and psychosocial interventions for the treatment of mental disorders.

Technical Work Based Skills

  • Work based skills (Dementia, Maybo competent) or willingness to develop in post.

 

  • Able to confidently undertake mental health and risk assessments and use of evidenced based supporting tool.

General skills / attributes

  • Skilled in independently coordinating clients in an agreed caseload.

 

  • Excellent communication and report writing skills, Typing/IT skills.

 

  • Adaptable and flexible to meet changing needs of the liaison team.

 

  • Excellent report writing skills and the ability to produce complex reports in a timely manner.

 

  • Ability to work on own initiative and organise own workload.

 

  • Knowledge of clinical governance and audit. 

Experience

  • Evidence of experience and ability to demonstrate sound application of skills post qualification
